Before your appointment & home blood pressure monitoring

We ask that before you attend your birthday clinic appointment to please record your weight and some home blood pressure readings. This is to aid your annual review and to also reduce the amount of time you spend in the Surgery.

If you are able to purchase a home blood pressure monitor we encourage you to do so. We recommend an arm monitor and not a wrist one.  However we do not wish people to feel under any pressure to do this if they are not able to and their care will not be detrimentally affected if they do not.

If you wish to purchase a home blood pressure monitor, guidance on home monitoring and a list of approved home monitors can be found below on the British and Irish Hypertension Society website

If you are unable to complete the form online or you cannot scan or photograph a completed version to email, the same information can be typed into an email and sent. 

Alternatively, if you are unable to email the results, please post these to the Surgery or bring the results to your birthday clinic appointment.

If your symptoms are deteriorating or you have any concerns, please make an appointment with the appropriate nurse or a doctor as well.
